This page contains links that will allow you to print our Required Office Paperwork, as well as an Insurance Form that contains a series of questions to ask your insurance company about your counseling benefits.  The Insurance Form is also extremely helpful to us, and we ask that whenever possible, our new clients download it and bring this completed form with them at the time of their first visit.  

If you have trouble printing either document, you can, of course, fill them out in the office before your first session.  If you choose to do this, we suggest arriving 15-20 minutes before your scheduled appointment time.

Once you have clicked on either link below, the form(s) will automatically open in Word® on your computer.  You can then print the documents by going to File on the menu bar, and then clicking on Print.
